Privacy Policy — Entry Point
Effective: [date the registration page goes live] · Company: MVMTLAB Inc., 28 av. de la Gare, suite 202, Saint-Sauveur, Québec J0R 1R0
This policy explains what personal information we collect when you register for the Entry Point seminar, why, who we share it with, how long we keep it, and your rights. It does not cover clinic patient records, which have their own policy.
1. Who is responsible
Dr Jeff Almon, chiropractor, is the person in charge of the protection of personal information at MVMTLAB Inc. Email: [email protected].
2. What we collect
- At registration: name, email, phone, profession, regulatory college or order, licence number, province of practice, chosen city and date, preferred language.
- At payment: Stripe collects your card details. MVMTLAB never sees your full card number; we receive your name, email, amount, date and the last four digits.
- At the seminar: attendance (sign-in and sign-out each day), your end-of-course evaluation, and any health information you choose to tell us before the practical labs.
- On the website: Cloudflare Web Analytics counts visits without cookies and without building a profile. No advertising cookies are used.
3. Why
- to process your registration and payment;
- to confirm you belong to a regulated health profession (a condition of admission);
- to send you practical information, your confirmation and receipt;
- to issue your certificate of attendance and keep it for your college's continuing education audits;
- to keep you safe during the practical labs;
- to meet our tax and accounting obligations.
We do not use your information for advertising and we do not sell it. We will write to you about future seminars only if you tick the box for it, and you can unsubscribe at any time.
4. Who we share it with
| Provider | Role | Where the data is |
|---|---|---|
| Stripe | payment and receipts | Canada and United States |
| Cloudflare | website hosting, cookieless statistics | United States and global network |
| Microsoft 365 | Canada or United States |
These providers are bound by contract and use your information only to provide their service to us. Some are outside Québec: before sharing information with them, we assess the protection they offer (Québec Law 25, s. 17).
We will confirm your attendance to your college only if you ask us to, or if your college asks during an audit and the law allows it.
5. How long
- registration and payment records: 6 years after the end of the fiscal year (tax obligations);
- attendance list and certificates: 5 years, to answer continuing education audits;
- health information given for the labs: destroyed no later than 30 days after the seminar.
6. Photos and video
No photo or video of you is taken or published without your written consent, given separately from registration. You can withdraw it at any time for the future.
7. Your rights
You can ask to see your information, correct it, or withdraw your consent. Write to [email protected]; we answer within 30 days. You can also complain to the Commission d'accès à l'information du Québec, or to your province's authority (in British Columbia and Alberta, the Information and Privacy Commissioner; elsewhere in Canada, the Office of the Privacy Commissioner of Canada).
8. Incidents
If a privacy incident creates a risk of serious harm, we will notify you, the Commission d'accès à l'information and the other authorities concerned.
9. Changes
We post any change on this page with its effective date.
